In-Patient Rehabilitation Admissions Process

The in-patient rehabilitation admissions process is a critical procedure designed to ensure that patients receive the necessary care and treatment upon entry into our healthcare facility.

When a patient arrives at our facility, they are typically greeted by their patient care coordinator who will collect essential information such as personal details, prescription medications, and any other important medical information.

Following this initial registration, patients may be required to undergo a series of assessments including medical history review, physical examinations, and diagnostic tests to determine the appropriate course of treatment.

Once the evaluation process is complete, the healthcare team will meet with the patient and any family members to review the personalized care plan to address the medical needs and concerns identified during the assessment or prior to admission.

Patients are then assigned a room and given a tour. Throughout their stay, patients are closely monitored by healthcare professionals who will administer treatments, medications, and therapies as prescribed by the attending physician.

Throughout the stay, patients and their families are kept informed about the treatment progress, prognosis, and discharge planning to ensure a smooth transition from the facility back to their home or a long-term rehabilitation facility if needed.
